    Health Action Plan Document

     A Health Action Plan is a document you can fill in about you and your health.  It includes details about your medication, if you have any allergies and how you communicate.  It is really useful if you take it along with you when you go to the doctors or if you have to go in to hospital.

    If you would like to complete one please contact Marie Gibb on 07748 920257.  Marie is keeping a list of people who have a Health Action Plan in Wakefield and will use this list to send updates and ask people what is working well and what isn't working well.

    Vulnerable In-Patient Card (VIP)

    The Vulnerable In-Patient Card is for people with learning disabilities.  It holds important information about you that doctors and nurses need to know if you have an accident or if you need to go to hospital in an emergency.

    If you would like a card contact Patient Advice and Liaison Service (PALS) on Freephone 0800 587 2108 or 01484 466172 or ask your local Community Team for Learning Disabilities.
