What we do

We are a team of 2 people who work together to support adults with learning disabilities and autism in Wakefield.  We also work with a bank of part-time facilitators on occasions.

We support individuals and work with their families helping them to express their own views, wishes and aspirations and make choices.  We also help them to plan for the future so they can live fulfilled lives.

We work with multidisaplinary agencies and families, checking that actions from an individuals plan take place, and changes within peoples lives are real.  We are also able to offer advice and guidance about person centred approaches ways of working.

How to Access the Service

You can have a person centred plan by making a referral.  We like to encourage self referrals and if you need to know more about how to do this you can contact Ken or Alison on the above telephone numbers and email addresses or you can complete the referral form on the right hand side and return it to the address on the form.
